Carrier co-location rates may be a percentage or a flat fee. Co-location is a VERY BIG business for the carriers and tower companies and they don't want to give up anything in most cases. This is determined on a case by case basis and there is no magic formula. The greater the leverage that you have, the stronger your bargaining position is. Co-locating for ground leases will vary from rooftop leases.

The question about cell tower lease rates is too general and depends on what your definition of "Upstate" New York is. If you live in Manhattan your idea of Upstate NY may be Orange County or Rockland, Putnam etc.
New York like most states has a lot of rural areas with several smaller metropolitan areas and population centers. The closer you get to a larger town or city, the better your chances of getting fair market value (at least $1,000 per month) on your cell tower lease. If your property or rooftop is the best location, or only game in town, your negotiation position is stronger, and your ability to maximize your values increase.
You also need to take into consideration land use and zoning issues of the are the carrier is trying to cover.
Are you negotiating the lease with a tower development company or with a wireless carrier agent? Are you near the interstate or surrounded by cow pastures for several miles each way?
These will all determine if your value will be $400 per month or $1000 or more.
Co-locating (subleasing) carrier revenue is difficult to determine based on your broad question.
At the end of the day, regardless of how savvy of a business person you are, if the carrier or tower representative knows you are not in the cell tower industry, you won't be on a level playing field in the negotiation process.
